Jim Milford, MD

Dr. Milford is a board certified family practice physician with Three Oaks Health in Johnson Creek and a long-time medical director with Rainbow Hospice Care. He completed his undergraduate work at the University of Wisconsin-Madison before receiving his M.D. from Loyola Stritch School of Medicine in Chicago. Among Dr. Milford’s many other professional roles have been that of Emergency Department staff physician (Watertown Memorial Hospital & St. Michael Hospital), Assistant Clinical Professor (UW Medical School & Marquette University),Volunteer at Rock River Free Clinic, and past Chief of Staff, Watertown Memorial Hospital.

Dr. Milford strongly believes that in order to be a good physician, one must advocate for patients, showing compassion and teaching along the way. In 2015, he was honored by the Wisconsin Medical Society with the Physician Champion Award for Advance Care Planning.

Cathy Hunter, PA-C

I received my B.S. in Exercise Physiology & Kinesiology from the University of Wisconsin - Madison. From there I went on to Marquette University to receive my Masters in Physician Assistant. I rejoined Dr. Milford at Three Oaks Health in October of 2018 after working with him previously for over a decade.

I love working in family practice due the wide range of patients I am able to see as well as the diverse health conditions I am able to treat. I, like many at Three Oaks, believe that the provider and patient are a team and should work collectively to achieve the patients goals. In the exam room I prefer the more informal style during visits in the hopes that it creates a comfortable, trusting environment for everyone.

I currently live in Oconomowoc with my husband, Matt and three wonderful kids, Grae, Rye and Matson.


Jenell Valenzuela, APNP

Driven by a passion for patient care and inspired by the expanded role of a Nurse Practitioner, I decided to advance my career to better diagnose and treat patients while incorporating essential nursing skills. I received a Master's degree in Nursing from Concordia University, becoming a Family Nurse Practitioner in 2022. Prior to this I was working as a registered nurse since 2011. I have worked in various hospital and home care settings, gaining knowledge and expertise in seeing the needs of patients during acute illness to needing management with chronic illness at home.  This blend of expertise is particularly beneficial in the field of Family Practice, where comprehensive care is essential to help each patient in their individual needs. 

My professional goal as a Nurse Practitioner is to treat, educate, and guide patients throughout their lives, fostering collaborative decision-making in health and wellness. This patient-centered approach ensures that each individual receives personalized and informed care. 

Personally, I grew up and currently live locally. I truly enjoy being in a smaller town. I love spending my free time with my family. My husband, Ruben, and I have two boys, Landon and Lucas. We also have one beloved dog, Pappy.  


Teresa Price, APNP

I grew up in Watertown, Wisconsin. I hold a Master of Science in Nursing from Alverno College, where I graduated with honors, and a Bachelor of Science in Nursing from Chamberlain College of Nursing.

With a preventative approach to healthcare, I work closely with my patients and their families to promote overall well-being. Before joining family practice, I gained two years of experience in heart and vascular services and spent six years working as a primary care provider consultant at a behavioral health hospital. Drawing from my background in behavioral health, I recognize the vital connection between mind and body and integrate this understanding into my practice.

Outside of work, I enjoy spending time with family and friends, gardening, distance hiking, exploring nature, and traveling. I am currently accepting new patients.
