Jim Milford, MD
Dr. Milford is a board certified family practice physician with Three Oaks Health in Johnson Creek and a long-time medical director with Rainbow Hospice Care. He completed his undergraduate work at the University of Wisconsin-Madison before receiving his M.D. from Loyola Stritch School of Medicine in Chicago. Among Dr. Milford’s many other professional roles have been that of Emergency Department staff physician (Watertown Memorial Hospital & St. Michael Hospital), Assistant Clinical Professor (UW Medical School & Marquette University),Volunteer at Rock River Free Clinic, and past Chief of Staff, Watertown Memorial Hospital.
Dr. Milford strongly believes that in order to be a good physician, one must advocate for patients, showing compassion and teaching along the way. In 2015, he was honored by the Wisconsin Medical Society with the Physician Champion Award for Advance Care Planning.
Linnea Nagel, PA-C
I received my B.S. in Biomedical Science from Marquette University in 2004 and received the Marquette University College of Health Sciences academic achievement award in Biochemistry. I graduated with a Master’s degree in Physician Assistant Studies from Marquette University in 2005.
My background in healthcare includes working in the areas of nephrology, cardiology, urgent care, family practice, internal medicine, and occupational health. I previously had a house calls program and I have a special interest in geriatrics and internal medicine.
Currently, I serve on the national Home Centered Care Institute’s Clinical Advisory team and am on the Editorial Board for the Arizona Center on Aging/Geriatrics Workforce Enhancement Program. I previously served on the board of directors of the American Academy of Home Care Medicine and I am a member of several PA professional organizations. I'm currently the Director of Clinical Education and a professor for the Marquette University Physician Assistant Program. I teach courses in evidence-based medical research, clinical decision making, and I oversee the summative clinical year curriculum.
In my free time, I serve as the Director of the Watertown Blue Revue Dance Studio and have been coaching the varsity team for over ten years.
Cathy Hunter, PA-C
I received my B.S. in Exercise Physiology & Kinesiology from the University of Wisconsin - Madison. From there I went on to Marquette University to receive my Masters in Physician Assistant. I rejoined Dr. Milford at Three Oaks Health in October of 2018 after working with him previously for over a decade.
I love working in family practice due the wide range of patients I am able to see as well as the diverse health conditions I am able to treat. I, like many at Three Oaks, believe that the provider and patient are a team and should work collectively to achieve the patients goals. In the exam room I prefer the more informal style during visits in the hopes that it creates a comfortable, trusting environment for everyone.
I currently live in Oconomowoc with my husband, Matt and three wonderful kids, Grae, Rye and Matson.
Jackson Milford, Business Manager
As the business manager for Three Oaks Health it is my responsibility to connect with our patients and aid in making their healthcare experience a memorable one. “Health” is a subjective term and each patient's needs are different. In 2013 I graduated from the University of Wisconsin - Madison with a B.S. in Dietetics. I have been in the health and fitness industry ever since, primarily working with chronic disease prevention and maintenance. Seeing the power of prevention and living a healthy lifestyle has exemplified even more that the health of an individual is based on their own needs and not predetermined statistics. The healthcare system as we know it needs to change to get patients the care that they want. It is with great excitement that I strive to be a catalyst for change in the coming years at Three Oaks Health. Please join us in the journey to make a change in healthcare.
Heather Webb, Medical Assistant
I have been a Medical Assistant for 10 years. I live in Helenville with my husband, two daughters, and my two Persian cats. When I am not busy working, I’d like to say I enjoy kayaking and hiking, but I really just like to sit in my recliner with a good book or watch a Netflix movie. I also enjoy doing counted cross stitch.
I am definitely a people person and love that my job allows me to interact with new and interesting people every day, as well as continue the relationships I’ve built with established patients. I strive to make each visit a positive experience for each patient. My greatest passion for my job is the relationships I have formed with families, which working in family practice allows me to do.
Sara Johnson, Certified Medical Assistant
I attended Madison Area Technical College and have been a Certified Medical Assistant in family medicine for 14 years. One of the best parts of working in a clinic setting is being able to build long term relationships and really get to know my patients. I strive to provide a positive experience every time.
I grew up in Stoughton and now live just outside of Cambridge with my husband and two daughters. I enjoy spending time with my family and being outside playing with my two Labrador retrievers. When I’m not attending band concerts or sporting events for my girls, I love going to flea markets and antique stores.
Liz Williams, LPN
I attended Madison Area Technical Collage Practical Nursing program and have been an LPN for the last 28 years. My nursing experience has been working in clinics. I have worked in clinics as a staff nurse as well as clinic management. I have joined Three Oaks Health after working with Dr. Milford for 13 years early in his practice and more recently supporting clinics and clinical staff.
I grew up in Lake Mills. I currently live in Watertown with my husband Andy and our Brittany Tony (aka Tony Stewart). I have three grown children Mark, Sarah, Shane and four grandsons, Austin, Dezmond, Logan and Levi. When I am not at work I enjoy gardening, camping, and spending time with my family.
My passion is in family medicine. I like working with people all age groups and health conditions. I enjoy getting to know patients and their families and helping them reach their health goals.
My special interests are in chronic care management, advanced care planning and being an advocate for patients and their families.
I like being a part of the care team at Three Oaks Health because we can all use our unique talents to care for patients.
Margaret Milford, Patient Service Representative
Being a Patient Service Representative at Three Oaks means that it is my job to make sure you have a fulfilling experience during your time at the clinic. Whether you need to schedule an appointment, have some concerns you want to discuss, or just need to pass a message along to Dr. Milford, I will try to provide as much help as possible. Talking and laughing with patients is one of my favorite parts about this job, so feel free to stop by the front desk and say hi!
I graduated from UW-Madison in 2016 with a B.S. in Environmental Sciences. After college I spent some time traveling to some amazing places but am now currently situated in an apartment in Madison. Some activities I like to do in my spare time are playing the piano, baking, and eating whatever I bake. You can often find me in the garden or curled up with my nose in a book, drinking a cup of tea.
Anne Thies, Patient Service Representative
I'm a Michigan girl at heart, but love the rolling green hills of Wisconsin and all the wild life. My husband and I moved here a little over two years ago. I love to spend time traveling and camping all over the country to see my family. I enjoy hiking, kayaking, fishing and an awesome book.
I graduated from Western Michigan University in 2001 with a BS in Psychology and Minor in Substance Abuse Services. My degree and experiences have equipped me with many skills for interacting with patients and their families. I love the Patient Service Representative role because I live to help others, whether it's to schedule your appointments, get a message to the clinical staff or just need some questions answered, my job is to help you however I can. I also have the ability to develop a positive rapport very quickly with all ages of patients and their families, in person or over the phone. I'm also coordinating patients for our newest service, Home Sleep Study and am looking forward to learning new skills to better serve our patients.
