Jim Milford, MD

Dr. Milford is a board certified family practice physician with Three Oaks Health in Johnson Creek and a long-time medical director with Rainbow Hospice Care. He completed his undergraduate work at the University of Wisconsin-Madison before receiving his M.D. from Loyola Stritch School of Medicine in Chicago. Among Dr. Milford’s many other professional roles have been that of Emergency Department staff physician (Watertown Memorial Hospital & St. Michael Hospital), Assistant Clinical Professor (UW Medical School & Marquette University),Volunteer at Rock River Free Clinic, and past Chief of Staff, Watertown Memorial Hospital.

Dr. Milford strongly believes that in order to be a good physician, one must advocate for patients, showing compassion and teaching along the way. In 2015, he was honored by the Wisconsin Medical Society with the Physician Champion Award for Advance Care Planning.

Cathy Hunter, PA-C

I received my B.S. in Exercise Physiology & Kinesiology from the University of Wisconsin - Madison. From there I went on to Marquette University to receive my Masters in Physician Assistant. I rejoined Dr. Milford at Three Oaks Health in October of 2018 after working with him previously for over a decade.

I love working in family practice due the wide range of patients I am able to see as well as the diverse health conditions I am able to treat. I, like many at Three Oaks, believe that the provider and patient are a team and should work collectively to achieve the patients goals. In the exam room I prefer the more informal style during visits in the hopes that it creates a comfortable, trusting environment for everyone.

I currently live in Oconomowoc with my husband, Matt and three wonderful kids, Grae, Rye and Matson.

Heather Webb, Medical Assistant

I have been a Medical Assistant for 10 years. I live in Helenville with my husband, two daughters, and my two Persian cats. When I am not busy working, I’d like to say I enjoy kayaking and hiking, but I really just like to sit in my recliner with a good book or watch a Netflix movie. I also enjoy doing counted cross stitch.

I am definitely a people person and love that my job allows me to interact with new and interesting people every day, as well as continue the relationships I’ve built with established patients. I strive to make each visit a positive experience for each patient. My greatest passion for my job is the relationships I have formed with families, which working in family practice allows me to do.

Sara Johnson, Certified Medical Assistant

I attended Madison Area Technical College and have been a Certified Medical Assistant in family medicine for 14 years. One of the best parts of working in a clinic setting is being able to build long term relationships and really get to know my patients. I strive to provide a positive experience every time. 

I grew up in Stoughton and now live just outside of Cambridge with my husband and two daughters. I enjoy spending time with my family and being outside playing with my two Labrador retrievers. When I’m not attending band concerts or sporting events for my girls, I love going to flea markets and antique stores. 


Jackson Milford, Business Manager

As the business manager for Three Oaks Health it is my responsibility to connect with our patients and aid in making their healthcare experience a memorable one.  “Health” is a subjective term and each patient's needs are different.  I am here to help accomplish your goals and needs whatever the case may be.  Please let me know if there is anything I can do for YOU!

In 2013 I graduated from the University of Wisconsin - Madison with a B.S. in Dietetics.  I have been in the health and fitness industry ever since, primarily working with chronic disease prevention and maintenance.  Seeing the power of prevention and living a healthy lifestyle has exemplified even more that the health of an individual is based on their own needs and not predetermined statistics.  The healthcare system as we know it needs to change to get patients the care that they want.  It is with great excitement that I strive to be a catalyst for change in the coming years at Three Oaks Health.  Please join us in the journey to make a change in healthcare.
