  • We can work with you should you need medication refills and advice;
  • When possible, I will offer home visits for patients who need to be seen before our clinic opens in January.

Hello from Three Oaks Health!

We are happy to report that construction has begun in our new space at 480 Village Walk Lane in Johnson Creek.

That means that Three Oaks Health is on schedule to open its doors on January 3rd, 2017.

I am excited about the beautiful space we have designed for you. And I'm proud to be offering primary care services that center around your vision of health.

Next time you find yourself on the corner of Hartwig and Village Walk, look for the bright banners in the windows of the strip mall (just under the Johnson Creek Water Tower).

Scheduling for January appointments:

While I am unable to see you in the clinic until the dust settles in January, I will be available on a case-by-case basis once I return in November. In preparation, you will soon receive a message with our phone number so you can reach me directly and begin scheduling appointments.

We recently updated our website to include a frequently asked questions page. Here you'll find many answers to the questions that you may have.

We are pleased to inform you we signed a lease for our new clinic in Johnson Creek at 480 Village Walk Lane. We will begin remodeling and plan to open our doors no later than January 2017.

We are in the process of establishing contracts with health insurers, and will update our website as developments occur.

We have also begun a mutually beneficial relationship with athenahealth, our electronic health record vendor. Soon you will be able to start registering and making appointments
